| Transport Information: | Deliver to lab immediately. All specimens must be signed into the laboratory. |
| Reference Client: | If unable to deliver to the lab immediately, store and transport at 2-8° C, except hair and nails (maintain at room temperature). Specimen must be delivered to lab within 24 hours. |

| Additional Information: | Fungal susceptibilities are performed by request only and the test is sent to an outside reference lab. The order must be placed in the computer. Notify the microbiology lab at 984-2358 in order to prepare the isolate for referral. |
